Back Side of Throwout bearing Bent?
Anyone ever seen anything like this before? I haven't ... This wasn't why I took the transmission out, either haha ...

This is purely speculation, of course, but those scratches on the lip look like someone installed is incorrectly.

Never seen anything quite like that.
Something experienced some serious trauma somehow. I'd be willing to bet if you looked at all the other parts, it's not the only one with damage.

I would assume the fork has to be banged up, too.
The transmission and sleeve that it rides on is fine. The clutch pressure plate fingers (from what I saw so far) look normal.

Maybe the throw on the clutch cylinder is too long. So when you push the clutch all the way in, the fork pulls too far back on the bearing, bending that ring in those 2 spots.

That bending is similar to something that happens when people put the spring tabs on the fork OVER the back of the bearing, and not in the groove ... but it's usually not that bad, and more chewed up. (The spring tabs were on the inside of the bearing where they belong anyways)

Mine looks even worse than that now. My pivot ball broke and I actually felt the bearing give. I'll post a picture when I get around to pulling it.

happens often with cheap bearing and stiff clutches on LT1 clutches but they are pull. cant say ive seen it happen on a push style clutch. im going with bad bearing or improper install.

Now I can say, from first-hand experience, improper fork geometry can do it in a heartbeat!
